Many many happy returns of the day is a salutation that has been used since the 18th century. While the phrase is still sometimes used to wish others well during the holidays, it is most often used to wish a person continuing good health on their birthday. The "returns," in this case, are the many additional birthdays the sender hopes you'll be celebrating.
